Reports to: Programs Director, Hepatitis Australia
Background Hepatitis Australia, incorporated in 1997, is the national peak community organisation to progress national action on issues of importance to people impacted by viral hepatitis.
Hepatitis Australia is a non-government, not-for-profit community peak body. Its members are the eight State and Territory Hepatitis Organisations.
Hepatitis Australia's vision is the elimination of viral hepatitis in Australia. Its ultimate goals are:
Expedite the elimination of viral hepatitis in Australia. Minimise the transmission of viral hepatitis in Australia. Maximise the health and social outcomes for people with viral hepatitis in Australia. Make an appropriate contribution to similar goals within the Asia/Pacific Region and globally. About the role As a Community Nurse Navigator, you'll provide telephonic health assessments, education, and support to people affected by viral hepatitis. You'll also:
Offer guidance and referrals to local services. Support clients, carers, and healthcare professionals. Promote the HepLink service through outreach and engagement. Collaborate with a national team and local hepatitis organisations. Help reduce stigma and improve access to care. This role is ideal for a skilled clinician who values compassionate, stigma-free care and wants to be part of a supportive, forward-thinking team.
What you'll bring We're looking for someone with: Current AHPRA registration as a Registered Nurse. 3–5 years' experience in community or public health nursing. Experience in viral hepatitis or liver health (highly desirable). Strong communication, critical thinking, and care coordination skills. A passion for health equity, advocacy, and client empowerment. Selection Criteria Qualifications and Experience Current AHPRA registration as a Registered Nurse. 3–5 years' experience in community or public health nursing. Experience in viral hepatitis or liver health (highly desirable). Proficient in telehealth platforms, electronic health records, and digital tools for remote service delivery. Client-Centred Care and Coordination Skilled in conducting safe, non-judgemental client assessments. Proven ability to manage complex cases and coordinate multidisciplinary care. Strong advocacy for client rights and access to timely, appropriate services. Experience contributing to quality improvement or research initiatives. Critical Thinking and Adaptability Strong problem-solving and decision-making skills in dynamic settings. Ability to work independently, adapt quickly, and manage competing priorities. Confident in delivering health education to clients, families, and professionals. Communication and Engagement Excellent verbal and written communication skills. Skilled in consultation, negotiation, and health promotion. Able to engage diverse audiences using a range of communication tools and channels. Teamwork and Collaboration Works effectively both independently and as part of a multidisciplinary team. Shares expertise and supports colleagues across the HepLink network. Willing to mentor and guide less experienced staff. Cultural Competency and Inclusion Demonstrated ability to provide inclusive, culturally safe care to people from diverse backgrounds, including Aboriginal and Torres Strait Islander peoples, people who use drugs, and culturally and linguistically diverse communities. Commitment to reducing stigma and promoting equitable access to care. Community Outreach and Health Promotion Experience in community engagement and participation in outreach or public health initiatives. Ability to promote awareness of viral hepatitis and support national health campaigns. Professionalism and Integrity Demonstrates initiative, accountability, and a commitment to continuous learning. Operates with a high level of independence and ethical practice. Applies logical thinking to navigate uncertainty and resolve issues. Contributes to and applies effective risk management strategies. Alignment with National Health Goals Understanding of and commitment to Australia's national strategies for viral hepatitis elimination and public health improvement.
