Permanent part time opportunity to apply high-level Social Work skills and knowledge in the delivery of psychosocial care to consumers and their families/carers of the Community Palliative Care service.

About the role

Permanent part time opportunity to apply high-level Social Work skills and knowledge in the delivery of psychosocial care to consumers and their families/carers of the Community Palliative Care service.

About you

Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:

- Deliver high level of Social Work clinical services including comprehensive psychosocial assessment, and evidence-based end of life care interventions across a wide age group spectrum, in accordance with prescribed qualified and ethical standards to consumers and their families/carers of Community Palliative Care service.
- Utilise high level communication and interpersonal skills to achieve optimal consumer and service delivery outcomes, in collaboration with consumers, their families, community service providers, other health professionals, supervisors and students
- Initiate and actively participate in quality improvement activities including assisting in the evaluation, review and development of the local service and its resources.
