Northern Coalfields Community Care Association Limited ('NCCCA') is a community based not for profit organisation who is an approved provider for residential aged and community care under the Aged Care Act 1997 and accredited community transport operator with Transport for NSW.


The Role

NCCCA has an exciting opportunity for a fulltime energetic and caring Commonwealth Home Support Program (CHSP) Assessment Officer that has the drive and motivation to support and enable older people to live independently in their own home, whilst maintaining a quality of life that maximises their potential.

As the CHSP Assessment Officer, you will be responsible for delivering effective and high‑quality care coordination, ensuring that services are managed to support clients in achieving their care goals. This position plays a key role in facilitating tailored care solutions for clients and their families within CHSP, with a strong focus on promoting wellness and reablement.

You will be accountable for assessing client needs, coordinating appropriate services, and fostering independence by encouraging clients to build on their strengths and capabilities. Through collaborative planning and service management, you will support positive outcomes that enhance quality of life and empower clients to remain active in their communities.

To be successful in the role you will have excellent communication, problem‑solving skills, be a high-level business administrator and hold a Diploma of Community Services (or be working towards). We are ideally looking for someone with experience in CHSP coordination and are seeking the right person that is passionate about working in community aged care and well versed in identifying, coordinating, and managing services.
