What we are looking for:
Serenity Mode is looking for dedicated and compassionate Support Workers to join our expanding team in Shepparton and Metropolitan Melbourne. In this rewarding, hands-on role, you'll provide essential support to participants with complex medical and/or mental health needs—assisting with daily living activities and empowering them to enhance their independence and overall wellbeing.
About you:
We're looking for someone who is compassionate, professional, and adaptable.
You'll have:
· A Certificate III (or higher) in Disability, Aged Care, Individual Support, or similar
· We are looking for support worker who has significant, proven industry experience.
. Fluent in English and exceptional communication skills
. Mental Health experience
· A current Working with Children Check and Driver's License (essential)
· Current NDIS workers screening check
· Strong communication, time management, and teamwork skills
· A positive, confident attitude and a genuine desire to support others
· Applicants who are willing to be flexible on days, times and location and have experience of working in SIL & SDA houses will be highly regarded by the company.
Key Responsibilities:
· Provide in-home support to clients with mental health challenges, promoting independence and wellbeing· Support clients with personal care, mobility, and daily routines
· Monitor or assist with medication
· Perform light housekeeping, meal prep, shopping, and errands
· Monitor client wellbeing and report concerns to healthcare professionals.
· Develop and follow individualised care plans in collaboration with clients, families, and health teams.
· Offer emotional support and encourage self-esteem, empowerment, and self-advocacy.
· Communicate with family members, healthcare providers, and other stakeholders to coordinate care.
· Accurately maintain documentation such as progress notes, care plans, and incident reports.
