About the role
SPIKE Connections' is seeking a motivated and experienced Support Coordinator/Psychosocial Recovery Coach to join our team in Launceston, Tasmania. This part-time role will involve providing person-centred, recovery-oriented support to individuals with psychosocial disabilities and complex support needs.
What you'll be doing
* Coordinate and implement individualised support plans that promote independence, community participation and quality of life
* Facilitate access to a range of community-based services and resources to meet the needs and goals of clients
* Provide psychosocial recovery coaching to empower clients and build their capacity for self-management
* Collaborate with clients, their families, carers and other service providers to ensure coordinated and holistic support
* Monitor and review client progress, adjusting support as needed to achieve positive outcomes
* Maintain accurate client records and documentation in line with organisational and legislative requirements
What we're looking for
* Relevant tertiary qualification in a disability, health or community services field (preferred)
* Minimum 2 years' experience in a similar support coordination or psychosocial recovery role (preferred)
* Demonstrated understanding of the National Disability Insurance Scheme (NDIS) and experience in navigating NDIS processes (preferred)
* Excellent communication and interpersonal skills, with the ability to build trusting relationships with clients
* Strong problem-solving, time management and organisational abilities
* Commitment to person-centred, strengths-based and recovery-oriented practices
* Current driver's licence and willingness to travel within our catchment area of North, North East, and North West Tasmania (currently including Devonport)
