Description:
We are currently seeking a Practice/Registered Nurse to join our team to provide a range of clinical services to CHADAC clients and to promote health education programs with a key focus to support the GP clinic.
Key Selection Criteria:
* Current registration with Australian Health Practitioners Regulation Agency (AHPRA)
* A demonstrated knowledge of Aboriginal communities and cultures and an understanding of the issues, particularly in the health area which are affecting Aboriginal people and a demonstrated ability to communicate sensitively and effectively with Aboriginal people.
* Collect and maintain accurate patient information and medical records using Communicare and other designated software programs.
* Consult with CHADAC staff and the General Practitioner to develop and implement protocols, procedures, systems, and client registers to support chronic disease management and clinical pathways.
* Demonstrated knowledge and experience in using computers and particularly with Microsoft Suite and a demonstrated understanding of medical software
* Participate in the development of strong and collaborative links with service providers (internal and external) to improve the health status of families and the community on a multidisciplinary basis.
* At least 2 years post graduate experience
* Demonstrated experience in providing acute and chronic care in a primary health care setting
Duties will include (please see position description for full details):
* Provide culturally appropriate primary health care, health assessment, early intervention, case management, health promotion and health education within a team environment.
* Organising & referring clients for Home Medication Reviews & supporting clients who attend the clinic for medication management
* Provide prompt and appropriate interventions to individuals and the community who have identified health problems to maintain or improve their health and well-being.
* Collect and maintain accurate patient information and medical records using Communicare and other designated software programs.
* Consult with Viney Morgan staff and the General Practitioner to develop and implement protocols, procedures, systems, and client registers to support chronic disease management and clinical pathways.
* Any other duties as directed by the CEO.
