About the role
The Preventative Integrated Care Service (PICS) will provide a rapid response (review within 24-48 hours) for intensive medical management, delivered by a specialised medical, nurse practitioner and allied health with a disease specific focus through flexible and varied modalities. This service addresses a gap for a target population of high-risk chronic condition patients requiring specialist intervention and coordinated care in their most vulnerable periods to improve quality of life, avoid unplanned admissions, prevent potentially preventable hospitalisations and reduce length of stay.
The purpose of this role is to deliver high level nutrition and dietetic clinical services and leadership to consumers of the Preventative Integrated Care Service (PICS), West Moreton Health Services (WMH). Duties are of a complex and varied nature and the majority are performed with a high level of independence.
About you
- High level of knowledge, expertise and skills in one or two areas of clinical practice
- Ability to communicate effectively and to work cooperatively with multidisciplinary teams including internal and external and stakeholders regarding service delivery and improvement opportunities.
- Ability to provide novel solutions to complex clinical problems through the adaptation or modification of established procedures
- Ability to initiate, plan and evaluate quality improvement initiatives.
- Ability to provide nutrition and dietetic education, clinical leadership and clinical practice supervision to less experienced Dietitians and dietetic students.
**Mandatory Requirements**:
- Vaccinated against, and remain vaccinated against, certain vaccine preventable diseases (VPD) as outlined in the Health Employment Directive NO. 01/16
- Vaccinated against COVID-19 as outlined in the Health Employment Directive NO.12/21and the Queensland Health Human Resources Policy B70.
