About Central Gippsland Health

Central Gippsland Health (CGH) is a sub-regional health service and the major provider of health and aged care services in the Wellington Shire.

CGH is located in the City of Sale which is 215 km east of Melbourne. The City of Sale is an ideal access point for the magnificent Gippsland Lakes, 90 Mile Beach and the Victorian Alps.

CGH provides acute care as part of an integrated service delivery model across Wellington Shire and beyond. The service provides beds across three campuses (Sale, Maffra and Heyfield).

Acute services include a full-time emergency department, critical care unit, neonatal critical care unit, three operating theatres, day procedure unit, oncology and dialysis services in addition to general medical and services and sub-acute services including rehabilitation.

CGH also provides aged care and community services in the Wellington Shire and beyond.

We operate four residential aged care homes located in Sale, Maffra, and Heyfield delivering high-quality, person-centred care in supportive and homelike environments.

CGH provides a wide range of community-based services that support people to remain healthy, active, and independent in their own homes. This includes the provision of Support at Home, Commonwealth Home Support Programs, and other in-home services tailored to individual needs.

Our integrated model of care is supported by a multidisciplinary allied health team offering physiotherapy, dietetics, occupational therapy, speech pathology and social work. We also provide accessible public dental services to enhance community wellbeing.

CGH is also a leading teaching hospital, affiliated with Monash University's School of Rural Health and provides placements and clinical experience for students.

About the Role

You are required to support the Maternal Child Health Service Team to improve the health and wellbeing of parents and children identified as being at risk of poor outcomes, by providing individualized, focused and intensive support.

Ensure families have a voice by providing advocacy in relation to social justice, human rights, and social change.

Provide a comprehensive and focused approach for the early intervention and prevention of physical, emotional, and social issues affecting young children and their families and ensure appropriate and timely referral for children and support for families.

Duties

Participate in Intake and Allocations, assess eligibility of client as per Enhanced Maternal and Child Health Program Guidelines.

Conduct initial assessment, identify presenting concerns, family history (including a genogram), establish a Child and Family Action Plan (CFAP).

Apply specialist knowledge and skills to identify and address the needs of children and families, in particular children identified at risk of harm.

Conduct risk assessments to identify and report children at risk of harm to Child Protection in consultation with the Enhanced Maternal & Child Health Nurse and Program Leader Enhanced Maternal and Child Health.

Qualifications, Skills and Experience

Demonstrated ability to work with families using strengths-based and family-centred principles of practice.

Recognition of differing social and cultural values and attitudes and a respect for client's self-determination.

Extensive knowledge in early childhood development and ability to recognise deviations from normal health and development and respond accordingly.

An ability to interact with and relate to a diverse range of clients with varying degrees of complexity and need, in an empathetic and non-judgmental manner, respecting the client’s rights, privacy, values and individuality.

An ability to work constructively within a multidisciplinary service system and develop team-oriented relationships with other service providers.
