About the role:
This is an exciting opportunity to develop your skills and make a real impact from day one. As a member of our Community Health Physiotherapy Team, you'll be helping patients stay active, independent, and well-supported in their homes and local community.
* You will contribute to multidisciplinary care on the ward and support timely, safe discharge planning.
* Provide personalised therapy sessions and home visits focusing on patients' goals.
* Lead group programs centred on recovery and self-management.
