Child and Family Practitioner Job Summary
We are seeking a skilled Child and Family Practitioner to join our team. The successful candidate will work with children, young people, and families in a family-based care arrangement, providing high-level support and supervision.
* This is a full-time position that requires the ability to develop and implement individualized care plans, prioritize the protection and well-being of children and young people, and maintain open communication with government agencies, carers, and stakeholders.
Key Responsibilities
1. Child-Focused Casework: Undertake case planning and assessment in partnership with relevant government agencies, children, and their families, carers, and other stakeholders.
2. Support and Supervision: Provide ongoing support and supervision to Foster Carers in relation to their caring for children and young people in their home, including support and supervision by telephone and through home visits.
3. Care Planning: Support and supervise Foster Carers in the case planning process, assist them to implement strategies outlined in the case plan, and ensure necessary risk assessments, cultural support plans, safety plans, and behavioral support plans are developed and implemented.
4. Skill Development: Ensure all necessary documentation and information, such as carer renewals, foster care agreements, file notes, and other records, are kept up-to-date, including inputting information into the CIRTS database.
Requirements
* A recognized degree qualification in Social Work or Psychology or related field.
* Significant experience in the child protection and out-of-home care sector.
* Demonstrated experience working with individuals, families, and communities from a strength-based, culturally appropriate perspective.
* Demonstrated experience identifying an individual's strengths and areas for improvement within the context of providing foster and/or host care with an emphasis on permanency for children in care.
* Current driver's license.
