Key Accountabilities, Responsibilities & Deliverables
Service Provision
· NDIS Support Coordination/ Recovery Coach: provide care and practical support to eligible individuals with an NDIS Plan to strengthen their abilities to coordinate and implement supports and participate more fully in the community.
· Counselling Service: assist in the development, delivery and evaluation of high-quality therapeutic services and maintain respectful and compassionate relationships with program participants.
· Case management: providing clinical supervision to less experienced clinicians or mental health support workers, including maintaining clinical standards and supporting clinical governance
· Provide services and supports in collaboration with others.
· Ensure that positive and productive relationships exist among LWWHC, the local community, and families.
· Demonstrate an understanding and ability to provide services and supports within the parameters of the funding available.
· Participate in regular professional development opportunities to keep up with current developments
· Fulfill administrative requirements such as data collection and record keeping
· When assigned, supervise and train assistants, volunteers and students, ensuring the maintenance of professional, clinical, ethical, and safety standards
· Ensure compliance with our duty of care to clients (Code of Conduct).
· Ensure services meet quality service standards, and contract agreements.

· Contribute to maintaining an effective team.
· Actively support the clinical and professional development of peers.
· Attend and participate in meetings, reviews and committees as required whilst seeking guidance and feedback from others for work performed.

SELECTION CRITERIA
Qualifications & Knowledge/Experience
· Bachelor degree or above in Social Work, preferred AASW registered
Technical Competencies
1.Demonstrated NDIS knowledge and ability to provide individualised practical support to clients and their families/carers in accordance with the client's NDIS goals.
2.Demonstrated skills and ability in engaging and working with clients from diverse demographic and professional background.
3.preferred: Demonstrated ability to apply therapeutic interventions, assessment tools and group facilitating under government funding schemes.
1. Demonstrated ability to work under KPI for employee model only
5.Excellent high level communication skills including verbal, written and interpersonal.
6.Demonstrated knowledge of cultural awareness and understanding of working with differences in culture and identity.
1. good computer skills and experience with Microsoft Office and management software.
Licenses & Accreditations
· Current NDIS Worker Card
· Current Blue Card
· Professional association AASW membership with Professional Indemnity Insurance (preferred)
· Driver's license and a car (required for all roles which travel to deliver services)
