About the Role
Central Coast Local Health District (CCLHD) is seeking registered nurses (permanent full-time/part-time and temporary) to join the Hospital in the Home (HITH) team at Gosford, Wyong Hospitals, and the Long Jetty Health Care facility. In this role, you will serve as a primary healthcare provider in the community, delivering acute clinical care to patients in their homes or HITH clinics. You will collaborate with a multidisciplinary team to monitor health, respond to episodic needs, and promote health maintenance, rehabilitation, and disease prevention.
Your responsibilities include providing acute clinical care in patients' residences or at HITH clinics, working closely with the multidisciplinary team. You will need to liaise with community and hospital sectors, applying evidence-based nursing practices effectively. Educating patients and families about their conditions, medications, and self-care strategies is essential to empower them in managing their health. Flexibility is important, as you will assess and respond to diverse clinical situations in home settings. You will also record data for service evaluation and contribute to quality improvement initiatives.
Qualifications include:
* Current registration as a Registered Nurse (Division 1) with AHPRA
* Valid current driver's license
* Recent experience in Acute Care Nursing or a demonstrated interest in it
* Strong time management skills and the ability to work both independently and within a team
* Ability to liaise effectively with other health professionals for case management and care planning
