- Northern Adelaide Local Health Network – Northern Mental Health – Salisbury
- Salary: $91,785 - $106,330 p.a. (pro rata) plus Superannuation and Salary Sacrifice benefits – AHP2
- Temporary Part-Time – working 26.25 hours per week, up to 30 June 2026

About the Role:

The Northern Adelaide Local Health Network is seeking a Social Worker to join our Hospital in the Home unit.

This position offers the unique opportunity to work with patients who require hospital-level care while maintaining the comfort and familiarity of their home environment. You'll provide comprehensive social work services, supporting patients and their families through complex health journeys whilst collaborating with multidisciplinary teams to ensure holistic care delivery.

Key Responsibilities:

- conduct comprehensive psychosocial assessments in home settings
- develop and implement care plans tailored to individual patient needs
- provide crisis intervention and emotional support to patients and families
- collaborate with medical teams, community services and support networks
- facilitate discharge planning and community resource connections

This position offers flexibility whilst maintaining meaningful professional engagement in a supportive, collaborative environment where your expertise will make a genuine difference to patient outcomes.

To be eligible for this position, you must hold an appropriate Degree or equivalent qualification which gives eligibility for full membership of the Australian Association of Social Workers.
