About This Role
Your new role:
As a Social Worker within the Chronic Disease and Illness Team, you will deliver evidence-based, patient-centred social work services to patients and families experiencing chronic and life-limiting illness. Working across acute inpatient services (respiratory, kidney and cardiac) and outpatient kidney services, you will play a key role in supporting patient autonomy, safety and continuity of care.
You will work as part of a highly collaborative interprofessional team, contributing to discharge planning, counselling, future care planning, and bereavement support, while continuing to grow your clinical expertise under professional supervision.
Key Responsibilities Include
* Delivering comprehensive psychosocial assessments, therapeutic interventions and discharge planning within an interprofessional team
* Supporting patients and families through chronic illness, palliative, end-of-life and bereavement care
* Managing a varied clinical caseload using effective prioritisation, documentation and reporting practices
* Liaising with internal and external services, including community, government, NDIS and My Aged Care pathways
About You
You are a compassionate, skilled Social Worker who thrives in complex clinical environments and is committed to delivering high-quality, patient-centred care. You bring strong communication skills, sound clinical judgement and a collaborative approach to supporting patients, families and multidisciplinary teams.
You Will Bring
* Experience or demonstrated capability in hospital-based or health social work practice
* Strong skills in psychosocial assessment, counselling and trauma-informed care
* Knowledge of complex discharge planning, including NDIS and My Aged Care pathways
* Ability to manage a diverse caseload in fast-paced inpatient and outpatient settings

The Division of Allied Health and Rehabilitation provides high quality, strong person-centred, evidence based, multi-disciplinary clinical, education and research services across the continuum of patient care.
