Employment Type: Permanent Full TimePosition Classification: Nurse PractitionerRemuneration: 139661.16 - 149538.56Hours Per Week: 38Requisition ID: REQ566662Advertising Closes: 5th May 2025Employment with Mid North Coast LHD requires candidates to hold a valid authority to work in Australia (Aust Work Rights Visa).What You'll Be DoingThe Rural Generalist Aboriginal Health Nurse Practitioner/Transitional Nurse Practitioner (NP/TNP) is a site-specific model providing high levels of clinically focused care across a range of services: emergency, inpatient, chronic and complex care, and primary care.The NP/TNP WillProvide holistic care to a broad range of acute and chronic presentations across the lifespan to address patient and community needs.Receive and make referrals to facilitate integrated care across disciplines or teams and contribute to clinical excellence through research, teaching, consulting, and providing leadership to the nursing profession.Work autonomously and collaboratively in an advanced and extended nursing clinical role, meeting the clinical demand within their defined and agreed scope of practice and formulary, including patients with chronic and complex care.Provide expertise in chronic and complex health conditions and support the development of best practice emergency/inpatient care plans and coordination of discharge plans for identified high-risk Aboriginal and Torres Strait Islander patients with chronic and/or complex medical diagnoses that present to Coffs Harbour Health Campus and Galambila Aboriginal Medical services.Create culturally safe partnerships to address priority population groups within the Coffs Clinical Network community including Aboriginal and Torres Strait Islander people, people living with a disability, people experiencing homelessness or on low incomes, older people, and culturally and linguistically diverse groups.Coordinate health care activity and referral to, and liaison with, internal and external community agencies and services relevant to their conditions.The NP/TNP role is grounded in the nursing profession's values, knowledge, theories, and practices and provides innovative and flexible health care delivery that complements other health care providers.The scope of practice of the nurse practitioner is determined by the context in which the nurse practitioner is authorised to practice.Diversity and Inclusion lies at the heart of how we recruitMid North Coast Local Health District is committed to building a diverse workforce that reflects and understands the community we serve.
