About The Team

You'll be part of the newly established New England North West Palliative Care Hub, based at Tamworth Community Health Service. This dedicated, multidisciplinary team is committed to delivering compassionate, high-quality palliative and end-of-life care across the region, supporting patients and families wherever they feel most comfortable.

About The Role

As the Clinical Care Coordinator - Palliative Care, you'll play a key role in leading and coordinating person‐centred care across the patient journey, from referral through to discharge. Working closely with nursing, allied health, medical specialists, volunteers and General Practitioners (GPs), you'll ensure care is seamless, timely and tailored to individual needs across both community and hospital settings.
In this senior clinical role, you'll support patients with complex needs through a mix of face‐to‐face and virtual care, while also guiding colleagues to deliver safe, compassionate practice. Your days will be varied and meaningful — from leading care planning discussions to supporting families through critical moments and helping shape integrated models of care.
You'll be supported by an experienced and collaborative team, with access to mentorship from senior clinicians including the Nurse Unit Manager (NUM), Nurse Practitioner (NP) and Clinical Nurse Consultant (CNC), alongside ongoing professional development through the Palliative Care Clinical Services Stream and online learning platforms.

About You

You're a confident clinical leader who remains calm under pressure and brings advanced skills in palliative and end‐of‐life care. You communicate with empathy, collaborate effectively, and prioritise what matters most for patients and families. A reflective practitioner and strong team player, you value connection, trust, and delivering care that truly makes a difference. You bring:

* Current registration as a Registered Nurse with AHPRA
* Postgraduate qualification in Palliative Care
* Minimum three years' experience in the specialty
* Current driver's licence and willingness to travel across the region
