We have an exciting opportunity for an Assessor in the Veterans' Home Care based in Blackburn.
In this rewarding office-based role, you will support veterans and their families through phone-based assessments and care coordination, working closely alongside a supportive team of colleagues.
What you'll do:
* Guide veterans and their families by explaining the Veterans' Home Care (VHC) program and available services – full training will be provided
* Manage inbound and outbound phone calls with veterans and their representatives
* Conduct structured phone assessments and reviews from the office, identifying care needs, personal goals, and ways to maximise independence
* Work collaboratively with your team and experienced colleagues to discuss cases and determine the most appropriate supports
* Recommend suitable VHC services and connect clients with community care providers
* Maintain accurate documentation in line with DVA program requirements
You'll have the chance to work independently whilst being part of a trusted team of people, who provide high quality support to the Veteran community. Play a crucial role in enhancing our Veteran's experience through empathy, warmth, understanding, and kindness.
What You'll Bring:
We're looking for someone who is organised and confident communicating with a wide range of people over the phone. Ideally, you will have:
* Minimum of two years' experience in the aged care or community care sector
* Strong customer service skills with a professional, empathetic, and collaborative approach
* Experience with assessments, care planning, service coordination, or phone-based client support (highly regarded)
* An understanding of the veteran community and how military service may impact physical and mental health (desirable)
* Excellent written and verbal communication skills
* Confidence using technology, including Microsoft Office
