**Where you'll be working**
Mona Vale Hospital Palliative Care and Geriatric Evaluation and Management Unit
**What you'll be doing**
Provide high level Social Work service to consumer's carers and families to ensure the provision of optimal health outcomes consistent with the districts and professional policies, procedures, standards, relevant legislation and Code of Conduct.
**_People of Aboriginal and/or Torres Strait Islander background encouraged to apply._**
- Demonstrate skills and knowledge in the provision of service to consumers, carers and families requiring assessment, counselling, crisis management, case management, advocacy, intervention, appropriate referral and discharge planning.
- Demonstrated ability to prioritise and manage workload demands, meet deadlines and complete activities (including ability to work within scope of practice and seek assistance when required).
