As an NDIS Support Coordinator, you will work with participants of the National Disability Insurance Scheme (NDIS) to help them understand, implement, and optimize their NDIS plans. You will assist participants in building the skills they need to manage their supports independently and connect them with appropriate services and providers.
Key Responsibilities:
* Work collaboratively with NDIS participants to implement their NDIS plans and achieve their goals.
* Assist in coordinating a range of supports including informal, mainstream, and funded services.
* Help participants understand their NDIS funding and how it can be used.
* Build participants' capacity to manage their own supports and make informed decisions.
* Liaise with service providers, community services, and stakeholders to ensure the effective delivery of services.
* Monitor progress toward goals, and assist with plan reviews and updates.
* Maintain accurate and up-to-date case notes, documentation, and reports in accordance with organizational and NDIS requirements.
* Identify and respond to risks, barriers, and crisis situations in a timely and supportive manner.
* Advocate for participants' needs and rights while promoting choice and control.
Key Selection Criteria:
* Relevant qualifications in Social Work, Community Services, Psychology, Disability, or a related field.
* Demonstrated experience working within the disability, mental health, or community services sectors.
* Strong knowledge of the NDIS and understanding of person-centered and strengths-based practice.
* Excellent communication and interpersonal skills.
* Strong organizational and time-management abilities.
* Ability to work autonomously and as part of a team.
* High level of computer literacy and experience with case management systems.
* Valid driver's license and access to a reliable vehicle (if required for the role).
* Valid Working with Children Check, NDIS Worker Screening Check, and Police Check.
