Overview

St Albans Home Caring is seeking a passionate individual to join our team as a NDIS Support Coordinator. The successful candidate will be responsible for delivering Level 1 and Level 2 Support Coordination and/or Therapeutic services to clients and their families. You will assist participants in reaching their goals, ensuring their supports are effective, and addressing various life challenges, such as health concerns or changes in circumstances. A solid understanding of complex issues, including medical conditions, homelessness, domestic violence, and the justice system, is essential. This position offers a chance for a driven individual to demonstrate their abilities in strategic thinking and planning skills. This role is ideal for someone who is bilingual that speaks both Vietnamese and English as the majority of our clients are Vietnamese.


Key Responsibilities

* Collaborate with NDIS participants, families, and other stakeholders to implement, monitor, and review NDIS plans.
* Facilitate the implementation of all supports outlined in the NDIS plan, including informal, mainstream, community, and funded supports.
* Connect participants with both formal and informal supports to improve their overall quality of life.
* Conduct and focus on managing participants' risks or crisis situations, identify and implement strategies to mitigate risks and support client's needs to meet their goals.
* Collaborate with service providers in the wider community to develop a network and ensure comprehensive care options for clients.
* Maintain accurate and up-to-date client records, ensuring confidentiality and professionalism.
* Work autonomously and as part of a small team, taking responsibility for assigned tasks and actively contributing to team meetings and planning sessions.


Qualifications and Skills

* Tertiary level qualifications in a relevant discipline (e.g. Disability, Individual Support, Community Services, Mental Health) or extensive/relevant experience in case management, counselling and or case work with families or people with a disability.
* Minimum 1 year experience in one or more of: Support Coordination, Support Connection, Local Area Coordination, NDIA Planning, Mental Health, Welfare worker, Counsellor, or Psychologist.
* Social Work, Mental Health Professional or Occupational Therapist are encouraged to apply.
* Demonstrated experience in the documentation of individual plans, risk management, incidents, daily case notes, staff reviews, performance management plans and reporting.
* AHPRA Registered Nurse with current registration status is preferred but not essential.
* Strong organizational and time management skills, with the ability to multitask and prioritize.
* Excellent communication skills, both written and verbal, with the ability to effectively engage with clients and service providers.
* Proficient in using various software applications, such as Microsoft Office suite and database management systems.
