Are you an experienced Nurse Practitioner with a passion for aged care? We're looking for a dedicated professional to join our Aged Care Services team in the stunning Lower Mid North Coast of NSW

Employment Type: Permanent Full Time

Remuneration: $143, $154,025.92 p.a + 12% superannuation + salary packaging

ABOUT THE ROLE

In this advanced practice role, you'll provide high-level, evidence-based care to older people, making autonomous clinical, diagnostic and prescribing decisions within your endorsed scope. You'll collaborate with geriatricians and the multidisciplinary team to design, implement and evaluate integrated management plans that improve safety, quality and experience of care. You'll also contribute to service planning, policy, research and quality improvement to grow the specialty.

* Provide expert clinical practice in Aged Care with the aim of early diagnosis, pre-emption, and reduced hospitalisation, utilising both home care and clinic-based services.
* Manage chronic & complex diseases of the older person such as cognitive impairment (including functional decline), dementia, sleep disorders, depression, anxiety, unexplained weight loss and falls.
* Meet the needs of aged and chronic care patients with a dynamic practice that incorporates the application of expert level of knowledge and skills in extended practice across stable, unpredictable and complex situations.
* Provide advanced support and guidance to the Residential Aged Care in Reach (RACI) program.
* Promote a standardised approach to achieve best practices for people with aged, chronic and complex health conditions such as improving care/service in the communities, health promotion and early intervention strategies.
* Implement a model of ambulatory care treatment for Aboriginal people experiencing Aged chronic and complex health issues in collaboration with Aboriginal medical services, Aboriginal Hospital Liaison Officers, Aboriginal Health Education Officers and the elders of the communities.

WHO WILL SUPPORT YOU AND THE SUPPORT YOU CAN EXPECT

You will be supported by the Service Manager of Integrated Care - Hunter & LMNC, Manager of Aged Care Services, Geriatrician/s, and Psychogeriatric team which includes a CNC & RN, along with members of the Aged Care Services team.

* Formal supervision and mentoring with the identified Geriatrician
* Operational leadership and support from Nursing/Service Manager
* Participation in communities of practice, policy, research and QI initiatives
* A safe workplace aligned to NSW Health Code of Conduct and HNE Health Values Charter

WHAT WE'RE LOOKING FOR IN A CANDIDATE

* Advanced clinical leadership in assessment, treatment and management in aged care
* High-level communication skills (coaching, written and verbal) and the ability to influence change
* Commitment to evidence-based practice, quality improvement and collaborative, multidisciplinary care
* Proven ability to prioritise in a dynamic environment and contribute to continuous service improvement
* Compliant with NMBA CPD standards and committed to ongoing professional development.
* A trusted clinical resource and mentor for nursing staff, providing leadership that lifts clinical care, shapes policy, and strengthens collaboration across all levels of the health
* Practises with professional efficacy: grounded in a nursing model, exercising autonomy and accountability as a senior member of the multidisciplinary team, and confidently using telehealth/video models where appropriate.

REQUIREMENTS

* Current authority to practice as a Registered Nurse with AHPRA and current endorsement as a Nurse Practitioner or, if transitional, evidence of substantial progress towards endorsement.
* The equivalent of three (3) years' full-time experience in an advanced practice nursing role within the area of intended practice, within the past six (6) years from the date when the completed application seeking endorsement as a Nurse Practitioner (Masters of Nursing-NP) is received by the Board.
