Leadership Role: Clinical Coordinator
This is a leadership role responsible for coordinating comprehensive resident assessments within a Positive Wellbeing Model.
* Ensure care plans are developed, implemented and reviewed in line with assessment outcomes and organisational timeframes.
* Coordinate the accurate collection and submission of clinical data to support evidence-based decision making.
o A thorough understanding of the Positive Wellbeing Model and its application in care planning is essential for this role.
o The ability to lead cross-functional teams and collaborate effectively with healthcare professionals is also required.
