Support Worker – Traralgon, VIC

About the Role
We are looking for caring, reliable, and motivated Support Workers to provide person‑centred support to NDIS participants in the Traralgon area. This role focuses on empowering individuals to live independently, engage in their community, and achieve their personal goals.
You will work in participants’ homes and in the community, delivering respectful, high‑quality support tailored to individual needs.
Key Responsibilities
* Deliver person‑centred support in line with individual support plans
* Assist with daily living activities such as personal care, meal preparation, and household tasks
* Support community access and social participation
* Promote independence, dignity, choice, and control
* Implement behaviour support and safety plans when required
* Provide medication assistance or prompting (where trained and authorised)
* Complete accurate progress notes and reports
* Communicate professionally with participants, families, and the care team
Essential Requirements
* Certificate III or IV in Disability, Individual Support, or similar (or willingness to obtain)
* Experience in disability or community support (preferred but not essential)
* Compassionate, patient, and professional approach
* Ability to work independently and as part of a team
* Strong communication and time‑management skills
Pre‑Employment Checks
Successful applicants will be required to complete the following prior to commencement:
* NDIS Worker Screening Check
* Working With Children Check (Victoria)
* National Police Check
* Right to Work in Australia
* Reference checks
* Qualification verification
* Current First Aid & CPR certificate
* High‑intensity support training (if required for the role)
* COVID‑19 vaccination evidence (as required by participants or organisational policy)
Vehicle & Insurance Requirements
As this role involves community access and participant transport, applicants must have:
* Current Australian driver’s licence
* Registered and roadworthy vehicle
* Comprehensive car insurance
* Business use insurance (required when transporting participants)
* Compulsory Third Party (CTP) insurance
* Completed vehicle safety checklist
