The Role
This position provides long-term intensive case management and service coordination with low individual caseloads to Aboriginal and/or Torres Strait Islander community members within the Barwon Region who are currently rough sleeping and/or experiencing chronic homelessness.
Key Objectives
* To provide culturally appropriate, safe, supportive and effective services in accordance with Wathaurong's values and legislative and funding requirements.
* To work under the Housing First principles and framework.
About the Job
This role is responsible for providing intensive case management and service coordination to Aboriginal and/or Torres Strait Islander community members within the Barwon Region. The successful applicant will be required to work under the Housing First principles and framework, ensuring that services provided are culturally appropriate, safe, supportive and effective.
