About The Role
Northern NSW Local Health District is seeking a Palliative Care Clinical Nurse Specialist Grade 2 (CNS2). This role works with members of the palliative care nursing team across inpatient and community settings to provide a consultative and liaison focused service aimed at supporting GPs, Community Nurses and the Residential Aged Care In-Reach Team in the assessment, care and management of patients with life limiting illnesses in the Clarence area.
The aim is to facilitate a coordinated, high quality palliative care service to the local community, with a case coordination model that involves generalist community nurses in providing direct clinical care and to use a multidisciplinary approach to care, with the end goal of improving quality of life for those patients and their carers.
