Description
A little about the role
Our Centre Based Program supports people living with a disability to reach their individual goals by providing unique and personalised support whilst having fun along the way. We provide quality support around our Excellent Care Framework encouraging participants to promote independence and build skills and achieve best individual outcomes. You will aid in achieving these outcomes by:
• Providing individual tailored support to participants to assist in achieving induvial goals and outcomes
• Supporting clients to participate in age appropriate activities and tasks including employment, education, recreation, adult living skills and community access and participation
• Making suggestions to improve service to participants and implement in consultation with the participant, family, representative and team
• Implementing individual communication strategies and assist in development and evaluation
• Working within a team environment to provide and implement our Excellent Care values to achieve best participant and team outcomes
• Working in accordance with all DPV Health policy and procedures including not limited to the National Quality and Safeguard Standards

• Are committed to supporting people with a disability to achieve goals and aspirations
• Have high level written and verbal communication skills
• Is flexible, committed and interested in career development and the strategic direction of DPV Health
• Holds relevant qualifications, however, someone with the right attitude and willingness to learn will be considered.
The fine print
• Successful candidates will be required to undergo a National Police Check, hold a valid WWCC and NDIS Worker Screening Check and be fully vaccinated against COVID-19
• Full driver's license and travel between sites is preferred
• Current Certificate for First Aid & CPR
