The role
Community Nurses is a team of compassionate Registered Nurses and Support Workers who are dedicated to provide NDIS participants with optimal care. Flexible and dignified support is provided in-home and in the community to individuals with a disability. We are building a support team for our Supported Independent Living accommodation to commence soon.

Responsibilities
- Meeting our clients' individual needs in the least restrictive way possible;
- Mentoring clients and assisting them to gain and develop skills.
- Being a positive role model and provide positive support to our clients' in areas such as emotional behaviour, transport, cooking and domestic duties;
- Assisting in dispensing client medication and adherence to client support plans;
- Completing clear, concise and factual handover notes and incident reports;
- Managing complex and challenging behaviors.
- Knowledge with diabetes management is an added advantage.
- Mental health support-problem solving strategies and social support.
- Supporting our clients to engage with their local communities and culture through access to Community Participation Programs and recreational activities;
- Adaptable to think "out of the box and positive self-esteem.
- Strong communication skills both written and verbal.
- Experience with high intensity support is an added advantage.
Mandatory requirement
- Current First Aid Certificate (mandatory requirement);
- NDIS Worker Orientation Module (mandatory requirement);
- Preferably previous experience supporting people concerns and needs;
- Basic computer literacy (training provided on our systems).
- Be willing and available for call in shifts.
- Comprehensive insured Car?
- Do you have a current NDIS Worker Screening Check?
- Do you have a current Australian driver's licence?
- Do you have prior experience as a disability support worker?
- Do you have a current Working With Children (WWC) Check?
- Have you completed a qualification in disability?
