Job Description
We are seeking a highly skilled individual to fill the role of Dementia Adviser. As a key member of our team, you will provide information, guidance and support to people affected by dementia in West Somerset.
Your primary responsibility will be to work across the West Somerset area, with the ability to cover other areas of Somerset as required. You will assist people with dementia and their carers in identifying their needs, providing information and guidance to help maintain their independence, improving their sense of well-being, and putting them in more control of their lives.
You will also build relationships with local contacts, network with health and care professionals, and provide dementia support. Additionally, you will develop networks and pathways with partner organisations who work in the field of dementia, ensuring seamless and consistent support and empowering individuals affected by dementia to make informed choices.
As a Dementia Adviser, you will provide support face-to-face, by telephone and online, and keep detailed and confidential records.

Required Skills and Qualifications
* Empathetic and non-judgmental approach with a commitment to equal opportunity.
* Knowledge and experience of Adult and Child Safeguarding, Information Governance policies and procedures and statutory laws such as the Mental Capacity Act.
* Good communication skills to meet the diverse needs of our community and represent their needs to statutory and other voluntary agencies.
* Sound knowledge of IT systems to record data, write reports and communicate by email.
* Understanding of client confidentiality and how this is applied when representing client needs.
* Ability and means to travel independently around Somerset.
