About the Roles:
We have two clinical opportunities available: a Chronic Disease Nurse who is passionate about supporting clients with chronic conditions, and a Child and Family Health Nurse to provide primary health care to Aboriginal children (0-6) and their families in a culturally safe and holistic manner.
As the Chronic Disease Nurse you will work closely with GPs, Practice Nurses, and the wider AWAHS health team, you'll assist with clinical assessments, chronic disease management plans, health checks and referrals to allied health and community programs. You'll help coordinate care across internal teams and external providers, support medication management, and contribute to case management for complex clients in partnership with the Chronic Disease and Outreach teams.
As the Child and Family Health Nurse you will conduct health surveillance and screening for children aged 0–6, and support parenting through education, counselling and group sessions. You are passionate about outcomes for children and families and is committed to providing life changing healthcare for children and young people for a healthier tomorrow.
Both these roles also involve Home Visits delivering early intervention and preventative health activities designed to promote community wellness, such as immunisations, health promotion and education, wound care, and assistance with minor procedures. You may also be involved in case conferencing, infection control, and health promotion events.

To be considered, you will need:
* Current APHRA registration as a Registered Nurse (Division 1)
* Relevant post graduate qualifications and clinical experience
* Demonstrated experience in delivering preventative health care, health promotion, clinical support, and minor procedures
* An understanding of the health issues impacting Aboriginal and Torres Strait Islander communities
* Confidence using Microsoft Office and electronic client management systems
* The ability to work autonomously and take initiative, while contributing to a collaborative, multidisciplinary team
* A current Working with Children Check
* A satisfactory National Police Check
* A current Driver's Licence
