Join our dedicated team as a Mental Health Clinician, providing holistic, evidence-based care to support recovery for individuals experiencing mental health challenges. Working collaboratively with Senior Clinicians and Team Leaders, you'll deliver comprehensive bio-psycho-social clinical assessments, treatment, and care plans that prioritise consumer centered and family inclusive practice.
**About the role**
Community clinicians are required to work a rostered function covering weekends to facilitate urgent assessments and support to consumers in their own homes, ED, medical wards and consumers from youth and aged services. This includes assessments over the telephone and medication management/support.
**Key Responsibilities**
- Provide evidence based and best practice clinical assessments, identify and prioritise consumers needs and develop risk management plans.
- Undertake comprehensive biopsychosocial assessments, identify and prioritise consumer and family needs, including risk assessment and management
- Attend Mental Health Tribunals and ensure consumers rights are met under the Mental Health and Wellbeing Act
- Co-ordinate crisis assessments and admissions to mental health bed based units.
- Monitor physical health conditions and promote physical wellbeing.
- Develop recovery plans in collaboration with consumers, families and carers, prioritise and action areas identified.
**Skills and experience**
- Recent clinical experience in a range of mental health settings, including bed based units and community mental health.
- Well-developed mental health assessment skills
- Good knowledge of the Mental Health Wellbeing Act and other relevant legislation
- Good knowledge of the recovery model and how this informs engagement with consumers
