Support young people to thrive outside of residential care by providing innovative, therapeutic case management through the Targeted Care Packages (TCP) program.

About the Program
Funded by the Department of Health & Human Services, Targeted Care Packages offer flexible, individualised support for young people at risk of entering (or transitioning from) residential care. The focus is on creating client-centered, sustainable living arrangements that promote stability and wellbeing.

What You'll Do
* Deliver trauma-informed case management tailored to each young person
* Support transitions from residential care to alternative placements
* Coordinate care plans and collaborate with families, carers, and sector professionals
* Provide outreach, advocacy, and ensure service compliance
* Manage individual care package budgets and reporting

What You'll Bring
* Qualification in Social Work, Community Services, or related field (or equivalent experience)
* Experience supporting vulnerable young people with complex needs
* Knowledge of trauma, attachment, and child development
* Strong communication and organisational skills
* Valid Driver's License + ability to pass WWCC and Police Check

Key Selection Criteria
* A relevant qualification in Social Work, Community Services or a related field, and/or experience in the community services sector
* A solid understanding of strength-based case management principles, particularly when working with young people and/or families with complex needs (e.g. homelessness, family breakdown, mental health)
* Strong understanding of trauma (and application of trauma-informed practice), disrupted attachment, and the developmental needs of children and young people experiencing complex challenges
* Excellent communication skills, with the ability to build trust and engage effectively with young people and families from diverse backgrounds
* Strong organisational and time management skills, with the ability to juggle competing priorities
* Knowledge of relevant legislation, including child safety standards, a current Driver's Licence, Working with Children Check and Police Check (or the ability to obtain them)
