Your Opportunity:

The Transition Care Program (TCP) is a jointly funded initiative of the Commonwealth Department of Health and Ageing and Queensland Health, offering time‑limited and goal‑oriented flexible rehabilitation care packages to older persons who have had a recent acute or sub‑acute hospital admission and live within the Wide Bay Health Service region. It is aimed at older clients, but age alone is not an eligibility criteria.


What we're looking for:

* Clinical Expertise - Demonstrated level of Occupational Therapy knowledge, clinical skills and expertise in a varied community and residential settings rehabilitation caseload, including clients over 65 or 55 for people who identify as Aboriginal and Torres Strait Islander.
* Team Participation/Communication - High level communication skills and ability to participate in an occupational therapy and/or multidisciplinary team involving staff, clients and their families/carers. Effective verbal and written communication, data entry and reporting and medico‑legal documentation.
* Clinical Ability - Demonstrated ability to apply established principles to routine contemporary occupational therapy clinical services and the ability to make more complex clinical decisions and solve problems under the clinical practice supervision or professional guidance of a more experienced practitioner.
* Leadership - Ability to provide and impart effective clinical skills acquisition, clinical practice supervision and determine competence, support and appropriately delegate tasks to Allied Health Assistants.


Preferred Qualifications:

* Mandatory possession of a qualification from a registered tertiary institution as an Occupational Therapist and Registration with the Australian Health Practitioner Regulation Agency with a current annual practising certificate is essential.
* Appointment to this position requires proof of qualification and if applicable registration or membership with the appropriate registration authority or association. Certified copies of the required information must be provided to the appropriate supervisor/manager, prior to the commencement of clinical duties.
* Aged Care: Potential applicants are advised that the Aged Care Act 1997 requires Queensland Health employees and volunteers of aged care services to have a current National Police Certificate. Queensland Health will facilitate the applicants obtaining the above check.
* Applicants in Aged Care are also advised that appointment to this position is subject to a Banning Order Register check, which will be conducted by the line manager to verify your eligibility to work in the aged care sector.

Our offer:

You will receive a competitive remuneration package value up to $132,567 p.a., comprising salary between $79,051 - $116,189 p.a., employer contribution to superannuation (up to 12.75%) and annual leave loading (17.5%) (HP3) (Permanent Full Time, 76 hours p.f.)
