The Clinical Nurse Specialist Grade 2 – Diabetes plays a pivotal role in delivering expert, patient‑centred diabetes care within a collaborative clinical environment. Under the direction of the Nurse Unit Manager – Diabetes, this position provides advanced clinical knowledge, leadership and education to support improved outcomes for people living with diabetes.
About the Opportunity
As the Diabetes Services Clinical Nurse Specialist Grade 2, you will deliver advanced, high quality diabetes education to people living with diabetes and their carers to enhance their understanding of diabetes, promote effective self‑management, support treatment adherence and encourage healthy lifestyle choices across inpatient and outpatient services throughout the CCLHD.
In this role you will have:
* Demonstrated advanced clinical leadership and specialist expertise in diabetes care, including the ability to conduct complex diabetes assessments, develop and implement individualised care plans, and provide ongoing management across the continuum.
* Be able to provide specialist guidance, education and mentorship to nursing and multidisciplinary colleagues, strengthening their competence and confidence in delivering high‑quality diabetes management and patient education.
* Proven ability to deliver evidence‑based, person‑centred and culturally sensitive diabetes care, advocating effectively for patients and empowering them to engage actively in their health journey.
* Contribute to the development and review of clinical practice, policies and procedures, and actively participate in quality improvement initiatives while maintaining accurate, compliant and contemporaneous clinical documentation.

About You
We are looking for someone who has:
* Current registration with AHPRA as a Registered Nurse (Division with relevant post registration qualifications and minimum three years' experience in the specialty area.
* Demonstrated advanced clinical expertise in diabetes management and the ability to deliver complex specialist care.
* Proven clinical leadership skills with experience mentoring staff and supporting development of nursing practice.
* Highly developed communication and interpersonal skills with strong information technology capability.
* Demonstrated commitment to professional standards, ethical practice, quality improvement and collaborative multidisciplinary care.
