This part-time position involves 15 hours per week, with potential for growth as caseload increases. You will work closely with clients to optimize their NDIS-funded supports, connect them with service providers and allied health professionals, and monitor their progress toward their plan goals.Responsibilities include:Assisting NDIS clients and their families in accessing and coordinating services.Developing collaborative relationships with stakeholders.Facilitating the implementation of supports outlined in NDIS plans.Enhancing clients' skills to support their coordination and community engagement.Empowering clients with understanding and implementing their plans.Conducting regular reviews of client plans and reporting to the NDIS.About you:To succeed, you will have:Excellent interpersonal and communication skills.Strong written communication abilities.Problem-solving skills.Effective collaboration skills.Knowledge of the NDIS.Minimum 2 years experience in NDIS Support Coordination.Familiarity with psychosocial disabilities.Additional requirements include:Current NDIS Worker Screening Check & Working with Children Check (or willingness to obtain).Current First Aid and CPR Certification (or willingness to obtain).Completion of NDIS Mandatory Training (or willingness to complete).A valid Driver’s License and a reliable, insured vehicle.Relevant qualifications such as Cert IV in Disability Services or Mental Health.How to apply:If interested and meeting the criteria, please apply via the Seek platform.
