Job Title: Care Coordinator for Individuals with Intellectual Disabilities

As a Care Coordinator, you will be responsible for providing expert advice to support staff in delivering patient/client care to people with intellectual disabilities. You will also provide care coordination and navigation for NDIS eligible individuals who are in hospital, at risk of hospitalization, or where the support will expedite access to NDIS services to relieve pressure on LHD services.

Requirements:
* Relevant qualifications in a health, disability, or social service role.
* Knowledge of intellectual disability and/or physical disability with current/recent experience working with people with intellectual/physical disabilities.
* Minimum 3 years' experience working in the disability sector, with a strong understanding of navigating the NDIS and NDIA.
* Experience coordinating and communicating complex care plans within a healthcare context, as part of a multidisciplinary team.
