Position: Permanent Full Time/Part Time/Casual Disability and Aged Care Support Worker
Location: Osborne Park | 15-35 hours per week

What You'll Do

Provide a premium care experience for our clients that is genuine, compassionate and personalised.

Assist clients with activities of daily living including personal care, meal preparation, light housekeeping, transport and companionship.

Deliver care in accordance with the clients care plan that has been developed by our Care Designers (Registered Nurse)

Meet the safety needs of the client and use equipment safely and properly

Complete documentation accurately and thoughtfully

What You’ll Bring

Work in a dynamic and supportive team

First Visits: Every time you visit a new client you will be supported by another team member

Electronic Care Plans

24-hour support by our Care Designers

Ongoing training: access to online training and support from our Care Designers

Flexibility - 24/7 care so shifts include day, evenings and weekends

Must have a minimum of 1 year experience as a Support Worker/Caregiver

Minimum Cert III Aged Care | Community | Disability| Individual Support

Right to work in Australia

Passionate about making a difference.

Drivers License and access to reliable transport in order to get to and from clients

Working with Children\'s Check

NDIS Worker Screen

NDIS Worker Orientation Module Completion

Current First Aid/CPR

Must be able to speak and write in English. Other languages will be an asset.

Ability to identify and report client care concerns to Care Designers when needed.

Ability to build relationships and have a genuine, care oriented nature.

Need to be prepared to drive at least 30 mins to get to client visits
