Job Description

* Part-time 0.8 FTE, max-term until June 2027
* Based in Ringwood on the lands of the Wurundjeri People
* CN4 classification + 12% Super + Generous Salary Packaging benefits of up to $18,550

The Co-ordinator plays a key role in the delivery of the Pathways to Good Health (PTGH) program, which delivers health navigation services for children aged 0–18 in Out of Home Care (OoHC). The program aims to promote health-seeking behaviours by connecting children to appropriate healthcare supports as early as possible.

Are you a values-driven Registered Nurse with a passion for improving health outcomes for vulnerable children and young people? If so, we’d love to hear from you!

We're looking for a dedicated Co-ordinator to join our Pathways to Good Health (PTGH) team. In this role, you’ll support the delivery of health navigation services for children aged 0–18 in Out of Home Care (OoHC) across Maroondah, Knox and Yarra Ranges.

Your focus will be on promoting health-seeking behaviours and helping to connect children with appropriate health supports. You’ll work closely with a multidisciplinary team, including paediatricians and allied health professionals, and foster strong collaborative relationships with key stakeholders such as Child Protection, ACCOs, and community services.

You’ll also provide support to the PTGH nurse, contribute to service reporting, monitor program performance, and help drive quality improvement initiatives.

As part of your day-to-day responsibilities, you will:

* Deliver services aligned with the National Clinical Assessment Framework for children in OoHC.
* Triage referrals and coordinate access to appropriate health pathways.
* Build meaningful relationships with children and young people to support health literacy and engagement.
* Collaborate with Child Protection and residential unit staff to ensure coordinated care.
* Support clinical governance activities, reporting and service improvement.
* Monitor and manage areas of clinical risk, including during outreach visits to residential units.

Who we’re looking for

You're a passionate and experienced Registered Nurse who thrives in a collaborative environment. You bring:

* A strong understanding of trauma-informed practice and the unique needs of children in OoHC.
* Demonstrated ability to coordinate with internal and external stakeholders.
* Sound clinical judgement and a proactive, flexible approach to service delivery.
* Division 1 Nursing qualification and current AHPRA registration.
* A current Victorian driver's license.
