Position Overview
The NDIS Coordinator is responsible for supporting participants to access services, manage their plans, and achieve their individual goals. This role involves coordinating supports, liaising with service providers, and ensuring compliance with NDIS requirements while promoting participant choice and control.
Key Responsibilities
* Participant Support
: Work closely with NDIS participants to understand their goals, needs, and preferences.
* Plan Implementation
: Assist participants in implementing their NDIS plans, including connecting them with appropriate services and supports.
* Service Coordination
: Liaise with internal teams, external providers, and community organisations to ensure seamless delivery of supports.
* Compliance & Reporting
: Maintain accurate records, prepare reports, and ensure compliance with NDIS guidelines and organisational policies.
* Advocacy
: Advocate for participants' rights and ensure they are empowered to make informed decisions.
* Capacity Building
: Support participants to build independence and strengthen their ability to manage their own supports.
* Crisis Management
: Respond to urgent participant needs and coordinate appropriate interventions.
Skills & Experience
* Strong knowledge of the
NDIS framework, policies, and procedures
.
* Experience in disability services, social work, community services, or case management.
* Excellent communication and interpersonal skills, with the ability to build trust and rapport.
* Strong organisational and time‑management skills to manage multiple participants and priorities.
* Ability to work independently and collaboratively within a team.
* Proficiency in Microsoft Office and case management systems.
