We are seeking a skilled Nurse to join our team in providing high-quality patient care and support. As a key member of our clinical staff, you will play a crucial role in delivering exceptional healthcare services to our patients.
About the Role
This part-time position offers an exciting opportunity to work as a driving force for preventative health in our local community. You will be responsible for supporting GPs and their patients in identifying and managing chronic conditions, with a focus on keeping them out of hospital.
Key Responsibilities
1. Care Planning: Prepare and implement care plans to help patients manage their diseases effectively.
2. Patient Empowerment: Coach patients on healthy behavior change principles to increase their ownership over their health.
3. Health Assessments: Conduct comprehensive health assessments to identify early signs of chronic disease.
4. Team Collaboration: Work closely with GPs to promote preventative healthcare throughout the practice.
5. Administrative Tasks: Manage administrative duties related to CDM appointments efficiently.
Requirements
To be successful in this role, you must have:
* AHPRA registration as an Enrolled or Registered Nurse.
* Proven experience in Chronic Disease Management and health assessments.
* Knowledge of Health Assessment is highly regarded.
* Strong organisational and time management skills.
* Outstanding communication and customer service skills.
