About the Opportunity
CCLHD have a unique opportunity for an experienced Occupational Therapist at Youth Health.
Based in Wyong, Youth Health supports the health and wellbeing needs of young people aged 12-24 years with multiple vulnerabilities and/or chronic and complex illness and disability. This is a unique opportunity to make a meaningful contribution as part of a dedicated youth specialist multidisciplinary team, working collaboratively alongside professionals in psychology, social work, nursing, medical and Aboriginal Health.
Professional supervision and peer support is encouraged within the team with strong links to the CCLHD Occupational Therapy Department.
This role will be 24 hours across a Monday to Friday week. Hours of work can be negotiated.
What you will be doing:
* Provide and review OT services within the Youth Health team, ensuring the provision of care meets the needs of young people accessing the service.
* Capacity building & goal-based therapy to improve the occupational performance skills of young people in a client centred model.
* Short to medium term intervention with care navigation alongside psychology, social work, nursing, medical and Aboriginal health workers in a multidisciplinary team.
* Delivery of care in a hybrid model of centre based and mobile visits using both individual and group-based intervention models.
* Develop and deliver programs to support young people with chronic illness or disability to manage their care and be as independent as possible.
* Support for families with complex needs to access supports and services in the community to build their capacity and support network.
* Collaborate with other professionals to deliver coordinated care and promote integration with other internal LHD and external services to ensure coordinated and integrated care pathways.
* Consultation to OTs both within the LHD and external to the organisations pertaining to specialised OT practice within youth/adolescent populations.
* Work alongside CCLHD OT department to provide specialist OT support to young people with chronic and complex health conditions and young people with multiple vulnerabilities.

About You
* You have a bachelor's or postgraduate degree in Occupational Therapy and registration as an OT with the Australian Health Practitioner Regulatory Agency with a high level of broad generalist and/or specialist knowledge within OT (paediatrics/youth/young adult).
* You are an experienced OT who is confident working independently and collaboratively within a multidisciplinary team.
* You are committed to providing effective, respectful, youth friendly and culturally safe practice to young people and their families and carers.
* Unencumbered driver's license and ability to travel across the district and to other external sites as required.
