Allied Health Clinician Level 3 - Transdisciplinary Physiotherapist - Quick Access Response Team - Wollongong Hospital
The Quick Access Response Team (QuART) is a dynamic community based allied health team providing interdisciplinary allied health care and care coordination to patients in their homes to prevent admissions or support earlier discharge from hospital. The position provides a specialised clinical service to patients across the Illawarra Shoalhaven Local Health District, delivering high quality outpatient and/or inpatient services and acting as a consultant for health professionals in Allied Health: Nutrition & Dietetics, Occupational Therapy, Physiotherapy and Speech Pathology. QuART clinicians work within an interdisciplinary model to provide 2 weeks of coordinated, at-home allied health support and intervention so patients can avoid hospital admissions or be discharged early. Clinicians work across allied health disciplines within the team and act as the lead for their discipline, liaising with Emergency Department clinicians, inpatient teams, outpatient and community services to coordinate ongoing care and support for patients. The team is located at Wollongong and Shoalhaven hospitals, with positions specific to each site, and may support both sites when required. Clinically, QuART predominantly works in the community visiting patients at home and provides a broad range of services, with some hospital-based involvement to facilitate discharges as needed.

Responsibilities
Provide a specialist clinical service to referred patients across Illawarra Shoalhaven Local Health District, delivering high quality outpatient and/or inpatient services.
Act as a consultant for other health professionals within the allied health discipline areas (Dietetics, Occupational Therapy, Physiotherapy, Speech Pathology).
Work within an interdisciplinary model of care and coordinate 2 weeks of at-home allied health support and intervention to prevent admissions or support earlier discharge.
Lead and coordinate service delivery for their discipline within the QuART team and collaborate with ED, inpatient, outpatient and community services to identify patients for the service.
Liaise with ISLHD outpatient/community services, GPs and external agencies to coordinate ongoing care and prevent hospital representations.
Provide care across Wollongong and Shoalhaven sites as required and contribute to hospital discharge planning when needed.
Requirements
Applicable vaccination requirements for a Category A role.
Working With Children Check (WWCC).
Current AHPRA Registration or eligible for membership of a professional association for disciplines not requiring AHPRA registration.
Valid NSW Driver's Licence.
Essential requirements include the ability to work in a temporary parental leave relief position until 13 March **** with potential extension or job share.

Employment details
Temporary Part Time 24 hours per week until 13 March **** at Wollongong Hospital (TWH). Salary: $***** - $***** per hour + 12% super & salary packaging. REQ. Grade: Physiotherapist Level 3, Diversional Therapist Level 3, Exercise Physiologist Level 3, Occupational Therapist Level 3, Social Worker Level 3, Speech Pathologist Level 3.
