About The Role
As a Senior Physiotherapist in the Transitional Aged Care Program, you will provide physiotherapy assessment and treatment for clients in the community. You will also contribute to client outcomes through case management, care planning and ongoing evaluation of service needs within a community-based model of care.
About Your Key Responsibilities
1. Assess and treat clients with various physical conditions, ensuring effective and safe practice.
2. Work collaboratively with multidisciplinary teams to develop and implement care plans that meet client needs.
3. Contribute to the development and implementation of quality improvement projects within the Transitional Aged Care team.
4. Maintain strong relationships with community services and health professionals to ensure collaborative care pathways.
About You
* You are currently registered with the Australian Health Practitioners Regulation Agency as a physiotherapist.
* You have experience working in advanced assessment and clinical reasoning within the aged care or community rehabilitation setting.
* You have demonstrated ability to provide clinical supervision and governance to junior physiotherapists and allied health assistants.
* You are committed to improving clinical practice and service delivery through leadership of quality improvement activities.
* You possess effective communication and consultation skills.
