About the Role
Our Support Coordinators work in collaboration with our Coordination Manager and stakeholders to enable our participants to break through barriers, with supported decision making and assistance to participate fully in their local community.
Role includes; implementation, connection and coordination of supports. Making effective, supported referrals to participant's preferred services, providing guidance to prepare for NDIS plan reviews and change of circumstances. Supporting individuals during crisis and complex circumstances. Working collaboratively across a variety of mainstream and community services. Assisting participant to create meaningful relationships and build their capacity.
About you
Qualifications, skills and experience.
* Significant previous experience in NDIS Support Coordination / Case Management years)
* Tertiary qualification - Allied Health (Occupational Therapy), Social Work, Counselling, Nursing or Human Services (and / or significant previous experience as an NDIS Support Coordinator)
* Ability to work in a team & individually
* Ability to build rapport with a diverse range of customers
* Experience working with customers with complex support needs
* Driven, energetic & committed
* Strong communication skills
* Excellent administrative and documentation skills including experience in using online portals and databases
* Strong connections with disability, mainstream and community networks
* Positive and friendly personality, who is open, flexible and willing to be part of an innovative and supportive team who work together
* Current driver's licence (essential)
* Mental Health expertise (desirable)
* Experience in either Acquired Brain Injury or Spinal Injury (desirable)
* Working with Children check and current police check (essential or willing to obtain)
* NDIS Worker Screen (essential or willing to obtain)
* NDIS Worker Orientation (essential or willing to obtain)
