Role Summary
South Western Sydney Local Health District is seeking a dedicated, outgoing and purpose driven Physiotherapist to join the Commonwealth Home Support Program team. This program provides an opportunity for Physiotherapists to work with a diverse client base in a community setting, focusing on rehabilitation in clients\' homes. You will work with families and the wider community to provide a holistic approach to client care.
This role involves working with clients who have a wide range of challenges and identifying goal-oriented rehabilitation plans in the home environment. You will have the chance to view clients\' lives holistically and support their rehabilitation goals.
You will be part of an Allied Health team and will be expected to build positive working relationships with other community services within the Local Health District.
There are one full-time and one part-time position, one based at the Fairfield Community Health Centre, and one based at the Camden Hospital. Please indicate your preference in your cover letter.
Responsibilities
Contribute to client rehabilitation in the home setting; collaborate with clients, families and community services; support holistic care planning; work within an Allied Health team to deliver patient-centred outcomes.
Selection Criteria
To help us assess your suitability for the role, please tell us as much as you can about yourself in response to the questions below.
* Current Physiotherapy Registration with the Australian Health Practitioner Regulation Agency (AHPRA)
* Demonstrated broad range of relevant physiotherapy clinical skills and experience in the geriatric population
* Demonstrated effective verbal and written communication, computer skills and time management skills
* Demonstrated ability to work independently and as part of a multidisciplinary team
* Share your experience in participating or leading a QI project, what the project was, what your role was and what was the outcome
* Applicants must possess an Australian C-Class driver's license (including P1 or P2). Those who hold a C class license (P1 or P2 included) from a state other than NSW must obtain a NSW C class (P1 or P2 included) license within three months of appointment. Learner\'s licenses are not permitted
