The Team:
The Tasmanian Health Service delivers the Aged Care Assessment Service (ACAS) South, providing both clinical and non-clinical assessments as part of the Assessment and Case Management Service. Our dedicated team is committed to delivering high-quality, person-centred care that supports older Tasmanians in accessing appropriate government-funded aged care services.
The Role:
Within the Tasmanian Health Service framework, a non-clinical Aged Care Assessor conducts Home Support Assessments through the My Aged Care platform. Using the Integrated Assessment Tool (IAT), the assessor identifies and evaluates an older person’s needs, goals, and preferences with a focus on wellness, reablement, and promoting independence.
You will need:
* Well-developed understanding of and demonstrated ability to undertake the functions of an Aged Care Assessor, including understanding the needs of older persons and the ability to plan and coordinate client care independently within a multidisciplinary team.
* Knowledge of the needs of older clients, the concepts of wellness and reablement, and the use of funded and non-funded services to meet client goals.
* Strong organisational skills and the ability to meet deadlines.

Details of appointment:
Permanent full-time, day worker position, working 76 hours per fortnight, commencing as soon as possible. Hours are negotiable with the successful applicant.
Applicants should note that, for twelve months from the publication date, this selection process may be used to fill similar full-time, part-time, or casual vacancies.
Salary: $81,915 to $94,263 per annum. Our Employer 11.5% superannuation contribution is on top of this amount.
Salary range is in accordance with the Public Sector Unions Wages Agreement 2022.

* Registration/licenses that are essential for this role must remain current and valid at all times during employment. It is the employee’s responsibility to ensure these remain valid and to notify the employer of any changes, revocations, or cancellations.
Desirable criteria include:
* Current Driver’s Licence
* Previous experience in case management, aged care, support work, care coordination, or individualised service planning.
