About the role
This position will deliver Lansdale House Accommodation Services in line with national youth services model of care, Journey to Independence. This position will provide quality client-centred support, and case management to young people aged 15- 18 years of age who are in the care of the department of child protection. Provide operational and client-based support in the running of this 24/7 accommodation service.
You will successfully
- Provide support to young people in line with Youth Services Model of care and associated Frameworks.
- Establish effective relationships with clients that encourage self-determination and participation.
- Ensure case management support is provided to clients in accordance with TSA case management models and standards of practice.
- Maintain accurate, thorough and up to date client records including case notes and data for all clients in accordance with TSA standards and procedures using TSA information systems.
- Ensure clients are assisted to access support services and establish community supports, including referrals to appropriate specialised agencies for counselling, therapeutic and remedial programs and where needed are assisted through advocacy with service providers.
There are multiple permanent full time positions available located at our Oasis House, Mirrabooka, WA. Salary and conditions are in accordance with the SCHADS WA Level 4 Award.
You will have
- Minimum Certificate 3 or 4 in Youth work / Human Services, and relevant equivalent experience in the Human Services Industry.
- Experience in working with youth in community or residential setting desirable.

- _ 395 checks_
- _ A national police record check is required (completed by TSA during recruitment)_
- _ First aid certificate (current or able to get within three months)_
