About The Facility
Major sites include University Hospital Geelong, McKellar Centre, Barwon Health North, and residential aged care facilities. Mental health, community health, and dental services are delivered throughout the Barwon South West Region, including Torquay and Colac.
About The Role
This is a short-term opportunity to join the interprofessional Connected Care team, providing care for clients with chronic and complex conditions in the community. Responsibilities include assessments, care coordination, service linkages, and some specialist allied health and medical care. The role is outreach-based, with most care provided in clients' homes via in-person visits, phone, or telehealth.
The team values a flexible, collaborative working environment focused on improving client outcomes.
