The Role
The NDIS Support Coordinator is responsible for assisting aboriginal participants to understand, implement, and manage their NDIS plans. This role focuses on building participants’ capacity to coordinate supports, connect with services, and achieve their individual goals in line with the NDIS Practice Standards and organisational values.
1. Plan Implementation
* Support participants to understand their NDIS plans and funded supports
* Assist in implementing and utilising plan funding effectively
* Monitor budget usage and ensure supports are delivered within approved funding
* Identify and address any gaps or risks in service delivery
2. Coordination of Supports
* Connect participants with appropriate service providers
* Establish and maintain service agreements
* Coordinate multiple supports to ensure cohesive service delivery
* Facilitate referrals to mainstream, community, and informal supports
3. Capacity Building
* Empower participants to make informed choices and exercise control
* Build participant skills in managing supports independently
* Promote self-advocacy and decision-making
4. Monitoring and Reporting
* Maintain accurate and up-to-date case notes and documentation
* Track participant progress toward goals
* Prepare reports for plan reviews as required
* Identify and escalate any safeguarding concerns
* Stakeholder Engagement
* Liaise with participants, families, carers, and service providers
* Work collaboratively with internal teams and external stakeholders
* Attend case conferences and planning meetings
6. Compliance and Quality
* Ensure services are delivered in accordance with NDIS Quality and Safeguards Commission requirements
* Adhere to organisational policies and procedures
* Maintain confidentiality and privacy standards
* Complete the reports required by funding bodies
Any ad hoc job as required for the day-to-day operation of the program.
About You
* Experience working with Aboriginal and Torres Strait Islander people
* Relevant qualification in Community Services, Social Work, Disability, or related field
* Demonstrated experience in NDIS Support Coordination or similar role
* Strong understanding of the National Disability Insurance Scheme (NDIS)
* Excellent communication and interpersonal skills
* Ability to manage competing priorities and caseloads
* Strong organisational and documentation skills
Desirable
* Identify as Aboriginal and/or Torres Strait Islander
* Demonstrated experience working with Aboriginal Community Controlled Organisations
* Knowledge of local service networks
* Experience using client management systems (e.g., MYP)
