**Job Overview:**
We are seeking a skilled Registered Nurse to join our Home and Community Care team in Perth, Western Australia. As a Registered Nurse with this organisation, you will partner with customers to assess needs, implement strength-based strategies and evaluate effectiveness.
**Responsibilities:**
* Assess customer needs in consultation with case managers
* Clinical Nursing assessments (PAS & KICKA)
* Cognitive assessments
* Develop, implement, and evaluate plans of care in partnership with customers and their significant others for both chronic and complex needs
* Liaise with healthcare professionals to deliver quality care to customers
* Participate in multidisciplinary team meetings
* Ensure compliance with organisation, professional and aged care standards
**Requirements:
To be successful in this role, you will need:
* Current registration with AHPRA as a Registered Nurse
* Ability to relate to elderly customers with empathy while working effectively with other health professionals, service providers and case managers
* Strong clinical assessment skills
* Work autonomously with strong communication, organisational and time management skills
* Enthusiasm for professional growth and development
* Competency with IDCs, wound care and strategies for living with dementia – desirable
* Cultural awareness and sensitivity
* Current Australian driver's licence and reliable vehicle - essential
**About the Role:
This is a permanent part-time role, working 30 hours per week.
