The Opportunity
The Next Step Plus Coordinator helps prepare young people (15yrs +) currently in care, or who have been a child in care, to develop the skills and knowledge they require to live independently. Working with indigenous and non-indigenous young people, this role will provide support and services to engage the person and promote positive outcomes specific to their individual requirements.
* Provide supports and services in response to the best interests of a young person to promote positive outcomes and assist the young person to achieve their goals.
* Provide a culturally responsive service and supports that meet the cultural needs of young people, particularly to ensure their ongoing connection or reconnection to family, community, culture and country.
* Facilitate the participation of young people in planning and decision making.
* Work with Child Safety and government and community stakeholders supporting young people in care (15+) in the development of a transition plan and gaining the skills, knowledge and networks they need.
* Identifying a natural mentor for the young person.
* Provide individualised support and help through resourcing and targeted responses based on an assessment of their needs.
* Develop and maintain relationships with significant family members, friends, community organisations and government agencies to promote lasting connections and positive outcomes for young people.
* Facilitate opportunities for the young person to participate in peer groups and networking enabling the young person to develop relationships with peers, learn from others, and build a social support network.
* Provide a continuum of support focused on transition from care planning and preparation for young people from the age of 15 years and continued availability of support up to the age of 25 years.
* Provide a strong localised, place based focus to promote a greater continuity of support for young people.
* Maintain cultural safe practice with the young person.
About you
* Ensure confidentiality of all information relating to children, families, and other agencies
* Always be well presented, friendly, courteous, and obliging when dealing with member of the community and colleagues
* Demonstrate commitment toward ongoing professional development
Must haves:
* Current Blue Card Holder or Willingness to apply for Blue Card
* Current Drivers Licence
* Cultural Competency would be highly desirable
